These Sites offer health information designed for educational purposes only. To make these Terms easier to read, Sites, our services offered are collectively called the “Services.” Please read these Terms of Service (the “Terms”) carefully because they govern your use of our Services. Please also refer to additional terms of service, informed consent and additional information in each section ("Planet") for specific information relevant to those sections. The following document provides general informed consent for all services and experiences we offer. This document aims to provide you with an understanding of what what we offer and what you can expect. Please read the information carefully and feel free to ask any questions or clarify any concerns you may have.

Purpose:

The purpose of our constellation of care  efforts is to provide you with safer and effective means of accessing information, services, referrals from the comfort and privacy of your own space.

 

Eligibility:

To be eligible for remote therapy, you must be located in a state where our services are authorized to be provided. Please note that we are only able to provide therapy services in accordance with the jurisdiction requirements of our professional licenses in Massachusetts and New York only.  Coaching and consulting is offered without geographic restriction/jurisdiction. You must also be at least 18 years of age.

 

Confidentiality:

Your privacy and confidentiality are of utmost importance to us. All information shared during any services will be kept confidential to the extent required by law. However, please be aware that there are certain limitations to confidentiality when utilizing electronic communication technologies. For example, it is possible that others may be able to overhear your session if you are not in a private location.

 

Nature and Limits of experiences:

Your experiences may involve physical, emotional, and psychological challenges. While every effort is made to ensure care and intentionality in our services, experiences and content, however there are inherent risks associated with engaging in these experiences. By participating in any experience with us you acknowledge and assume these risks.

 

Waiver and release of liability:

You acknowledge that you have read and understood the information outlined in this disclaimer. You understand and accept the risks associated with engaging in therapeutic experiences and agree to release thecervitudecircle.com and all its endeavors and information provided, including cervitudecounseling.com and cervitudecoalition.org, its owners, employees, contractors, volunteers, board members or third party information providers from any liability for any injury, harm, or damages that may occur as a result of your participation in any of our services, experiences and passive consumption of information or use of resource shares.

 

Fees:

Our fees for any services are based on the length of the appointment and the qualifications of the service provider. Payment is due at the time of service and can be made via credit card or other approved payment methods (including insurance if applicable). We reserve the right to change our fees at any time but will provide notice of any fee changes in advance. You acknowledge the financial obligation to pay out of pocket for these services.  Please refer to the specific service agreements and sections ("Planets") for additional information. 

 

Cancellation and Rescheduling:

If you need to cancel or reschedule any appointments, please provide at least 24 hours' notice if at all possible. If you do not provide notice or fail to show up for an appointment you will be held to the agreement with that specific service. 

 

Termination:

We reserve the right to terminate services at any time if we determine that they are no longer helpful or appropriate for you or if you violate any of our policies.

Consent:

You acknowledge that you have read and understood the information outlined in this section. You consent to participate in remote therapy and waive certain provisions of HIPAA related to remote therapy. You understand the risks and benefits of remote therapy, as well as the limits of confidentiality and security associated with the use of electronic communication technologies. You also understand that you may withdraw your consent at any time by contacting your therapist.
